Circle of Excellence Guidelines

Award Qualification for attendance at the Circle of Excellence event is open to all contracted Mutual Med agencies. Fully insured health premiums and ancillary products will be counted toward the trip qualification. All business with January 1, 2010, through December 31, 2010, effective dates and still in effect as of December 31, 2010, will be credited for the 2011 Circle of Excellence trip. Mutual Med calculates the trip qualification utilizing the point system below. Part D, short-term, blue transitions, basic and standard individual medical plans do not qualify for any points. A minimum of five cases must be written to qualify for the trip.

Additional Guidelines

• An agency can qualify for a maximum of two trips. Those agencies that earn enough points to qualify for more than two trips will be entitled to special awards determined by Mutual Med in addition to the two trips that were earned.
• The Internal Revenue Service requires a Form 1099 to be issued to each individual award winner.
• Mutual Med monitors the sales incentive points and reserves the right to exclude or disqualify any agent or agency whose conduct or business does not meet the ethical standards of Mutual Med.
• No Ancillary Product case may count more than 25 points toward the qualifying total.
• Mutual Med reserves the right to not award trip(s) if corporate retention and positive net gain goals are not met on group and/or individual business.
• If the winning broker cannot attend, Mutual Med reserves the right to award the prize to another broker in the same agency or to another agency at its sole discretion.

• Valid passport required.
• Bonus points will be awarded if more than 500 group health lives are in force and retention with Mutual Med exceeds 92%.
• Mutual Med reserves the right to modify trip point guidelines at our sole discretion.

• 400 Points - One agent and guest will be invited.
• 800 Points - Two agents and guests will be invited.
